At ianacare, we’re on a mission to support family caregivers with innovative solutions.  We partner with employers and health plans to provide comprehensive support that reduces caregiver stress, improves well-being, and enhances productivity. In 2025, we launched our partnership with healthcare providers on the CMS GUIDE Model to support caregivers of patients with dementia.  Designed to integrate with existing care ecosystems, our GUIDE product offers a dedicated, adaptive experience that empowers caregivers by streamlining coordination and delivering essential support.

We’re in an exciting growth phase and are looking for a passionate and organized person to join our team, drive successful partnerships within the healthcare sector, and manage new and existing employer clients.

About the Role:

As a Care Navigator, you’ll be the expert guide for senior caregivers navigating non-clinical support and benefits (e.g., Medicare, VA, employer-sponsored programs, and community resources).  You’ll build trust-based relationships, develop personalized action plans, and empower caregivers through phone, video, and chat interactions across six pillars: mental health, physical health, finance/legal, care management, work-life integration, and basic needs.

WHAT YOU’LL DO:

Conduct 1:1 video and phone sessions (30 min) and chat to assess caregiver needs and goals
Develop and update individualized care plans focused on support and benefits navigation
Guide caregivers to entitlements, community services, and employer-provided programs (e.g., respite funding, adult day care)
Collaborate with interdisciplinary teams (e.g., social workers, case managers, primary care providers) to coordinate non-clinical support
Document encounters, action plans, and resource referrals in our platform within 24 hours of each interaction
Monitor caregiver engagement; proactively reach out when participation drops or new needs emerge
Provide coaching via webinars, written resources, and videos to reinforce best practices in dementia caregiving
Provides follow-up with clients and family to help ensure access to services and programs
Maintains individual case records and complete documentation in addition to follow-up tasks
Become a Certified Dementia Practitioner within the first 90 days of employment
Conduct a variety of dementia education/support programs

WHAT YOU’LL BRING:

Demonstrates empathy and understanding of the challenges faced by caregivers and the loved ones they care for
You are a do-er, you live and breathe in to-do lists, and you are an overall proactive and organized individual with excellent project management skills
Possesses strong written and verbal communication skills, with the ability to clearly and effectively communicate with caregivers and interdisciplinary team members
Team player with a collaborative mindset and the ability to work effectively across different functions
Demonstrated resourcefulness and ability to troubleshoot or problem-solve independently
Comfortable using multiple tech platforms and navigating between systems
Comfortable working at a scrappy startup and managing multiple priorities
Detail-oriented and committed to delivering high-quality results
Proven expertise navigating senior benefits (Medicare, Medicaid, VA, LTC insurance, employer programs)
Commitment to HIPAA compliance and maintaining caregiver confidentiality
Strong documentation skills and comfort with tech tools (Zoom, Excel/Sheets, EHRs, chat platforms)
Ability to self-manage in a structured remote work environment, with a reliable, quiet workspace and consistent availability during shift hours
Ability to travel to headquarters biannually

PREFERRED QUALIFICATIONS & EXPERIENCE:

2+ years of professional experience in dementia care and geriatrics, or related non-clinical support roles (required)
Associate’s or Bachelor’s degree in Social Work, Nursing (LVN/LPN, RN), Case Management (CCM/CMC), or Community/Public Health Worker (MA, CHW, CHES)
Prior experience in telehealth, virtual coaching, or remote support roles
Familiarity with public and private benefits navigation (e.g., Medicare, Medicaid, VA, long-term care insurance)
Experience working with diverse populations, including those with limited English proficiency, low digital literacy, or socioeconomically disadvantaged backgrounds
Training or certification in dementia-specific communication techniques (e.g., Teepa Snow, Positive Approach to Care, Alzheimer’s Association programs) is a plus
Bilingual in Spanish or Mandarin is a plus and strongly valued in serving diverse caregiver communities
Location preference: Remote within the Eastern or Central Standard Time Zones

SHIFT STRUCTURE & COVERAGE

ianacare provides a 24/7 caregiver helpline, in addition to core navigator coverage from 8:00 a.m. to 8:00 p.m. ET, Monday–Friday. Navigator shifts are scheduled within this window as either three 12-hour or four 10-hour blocks a week to ensure reliable daytime support for caregivers.

Due to the nature of this role, availability during your assigned shifts is essential. Candidates must be able to commit to a consistent schedule within this time frame—flexible or ad hoc hours are not available for this position.
